Parents must complete or update the Vital Health Record in their child/children’s Magnus Health account PRIOR to arriving for the physical.  They will also need to print and bring a copy/copies of the Physical Examination Form from their Magnus Health account.  This form will then be completed by the examining physician.

There is no registration required for this event.  These will be the only dates and times Cary Academy will hold group athletic physicals for the 2013-14 school year, therefore it is recommended that you take advantage of this opportunity.  Please see the attached flyer for details and addresses of the offices.
